HMHP's Electronic Health Record system will provide better care
MyChart is a part of HMHP's new Electronic Health Record system, called CarePATH. We are introducing this new system to provide better coordinated care for you and your loved ones. CarePATH will eventually do away with paper charts and create an Electronic Health Record for each patient. MyChart allows you to access your continuously updated medical information as a part of CarePATH. 

Access your health information anytime from any internet connection
My Chart is free and easy to use. Your current health information is just a click away with MyChart – day or night, whenever you have access to the Internet. If you are a patient of a HMHP Physician Associates physician, you can log on securely to access continuously updated information through MyChart. This exclusive healthcare management tool is password-protected for your privacy and security.

View your medical information online
On MyChart you can access your health summary, view your immunization record, see a list of your medications, and even view some test results without having to wait for a phone call or letter. You can request prescription refills on MyChart without needing to place a phone call.  

 Manage appointments and request prescription refills
MyChart lets you request and cancel appointments online, anytime. You can also request prescription refills using MyChart without needing to place a phone call. With MyChart, you will not have to wait for office hours to schedule an office visit. Details of your upcoming or past appointments are also available through MyChart.   
 
Communicate with your doctor’s office securely
MyChart allows you to send private messages securely. It is easy to send a message and request non-urgent information from your doctor’s office. Your doctor’s office will be able to reply to you through MyChart to communicate with you privately and securely. MyChart is not for when you need an answer right away. If you are experiencing a medical emergency, please call 911.

 Log on securely with password protection
When you use MyChart, your information is safe from unauthorized access because all data is encrypted and password-protected with an individual user ID and user-controlled password. Your information is delivered via an encrypted connection. You can communicate safely with your doctor’s office by using MyChart. Unlike e-mail, all messages are securely transmitted through MyChart.

Access your family’s records
You can use MyChart to connect to your family’s health information. You can access a family member’s record by linking your account to theirs. Ask a staff member at your doctor’s office for details on how to sign up to access your family’s records. By linking accounts, you can access your family member’s appointments, immunization records, growth charts, and more.


Benefits of Electronic Health Records at your doctor’s office
In the doctor’s office, CarePATH, HMHP's Electronic Health Record system, will provide for better coordinated care. Your physician will be able to view your vital signs, test results, recent symptoms, and past treatments on the exam room computer screen. The Electronic Health Record system will give your doctor important details for prescribing medication, ordering procedures, or scheduling tests. Your medical history, allergies, special care instructions, and drug interactions are also available to the members of your care team.
 

Register during your next doctor’s office visit
When you arrive for your next appointment, ask a staff member about signing up for MyChart. It is quick and easy to sign up. You will select your own User ID and Password. You can write your User ID and Password on an ID card provided at your doctor’s office. Keep your MyChart ID card in a secure place where it can only be accessed by you. After you sign up, you can log on anytime with your ID and Password at MyChartHMHP.org.
